• health history and eye examination results
• billing or purchase information

We collect your personal information in order to provide you with safe and appropriate eye care.

Besides our staff, we may share certain information with:

• your medical centre if you are seeing us for a diabetic retinal screening 
• another optometry practice if they request the information on your behalf
• another health provider if requested as a result of a referral
  
  

Providing information is optional. If you choose not to share requested information, we possibly may be unable to provide the services requested.

We keep your information safe by storing it safely and only allowing appropriate staff members to access it.

You have the right to ask for a copy of any personal information we hold about you, and to ask for it to be corrected if you think it is wrong.
